Qualifications for Test Purchase
The substance abuse measures in this catalog are carefully developed
assessment instruments that require proper administration, scoring,
and interpretation. Therefore, eligibility to purchase, administer, and/or use
these measures for clinical purposes is limited to individuals
with training and experience in the area of assessment.
The SASSI may be used by:
1. Human service practitioners whose certification and/or
professional training includes assessment (such as psychologists, social
workers, certified addictions counselors); or
2. Individuals who have completed
authorized SASSI training.
Individuals who do not have professional training can administer and score
the instrument if there is appropriate supervision.
(It is suggested that you contact your state or provincial government
for specific information on the laws regulating assessment and testing
in your state or province.)

Qualification Form
Initial orders require a completed
Qualification form. The ethical guidelines of
the American Psychological Association require The SASSI Institute to
have this information at the time we fill your first order. For telephone
orders, please have this information available for the customer service
representative taking your order. For mail and fax orders,
please print and send the completed Qualification
Form with your order. Fax to 800.546.7995.
For online orders, simply complete the Qualification form in our
product catalog before checking out.
Returns
Damaged or defective materials may be returned for exchange
or credit at any time. Materials not defective in manufacturing or
shipped to you in error may be returned for exchange or credit within 30
days of purchase. Only unopened/resalable materials will be

Preview Option for Starter Kits
Starter kits may be examined for 30 days, at which time payment is due.
If you decide not to keep the kit, all materials, including manuals, user's guides,
scoring keys, unused questionnaires and profiles, and any videos or software included
in the kit must be returned for full credit. You may obtain a Starter Kit for a 30-day
preview by calling customer service at 800.726.0526.
Shipping
Orders received before 5 p.m. EST will be shipped the next business day.
Orders are shipped via United Parcel Service (UPS) ground. Allow 3 to 5
business days for regular ground. Rush orders are shipped by UPS (Next day
air, 2nd day air, or 3 day select) for an additional fee. For rush orders

All Alaska and Hawaii orders will be shipped via USPS Priority unless otherwise requested.

tests and related materials in any way whether the reproductions are
sold or furnished free for use is a violation of federal copyright law.

Product Information
Computer Software
The SASSI software is a Windows-based program that must be installed
on the hard drive of an IBM or compatible PC and operated within the Windows environment.
The minimum system requirements are:
- Pentium class processor, 133 MHz or higher (200 MHz recommended)
- 32 MB RAM (64 MB RAM recommended)
- 48 MB free hard disk space
- CD-ROM drive
- Windows 95 or higher
- Adobe Acrobat Reader 5.0 (included on SASSI CD)
SASSI software is available in three different "Types:"
Type 1 - The client completes the SASSI at the computer,
which graphs the results and gives a brief interpretation of
the client's profile.
Type 2 - The client completes the paper version of the SASSI.
The clinician then scores the SASSI and enters the client's scale scores
into the software to get a graph of the results and a brief interpretation
of the client's profile. (Paper tests and profiles are included with all
Type 2 purchases).
Type 3 - The client completes the SASSI at the computer, which generates
the outcome of the decision rules and a graph of the scale scores.
Prices for Types 1 and 2 are listed in the Product Catalog (for Type 3 pricing,
please call customer service). Volume discounts are available. In cases where
multiple users in the same organization will be installing the software on
different PCs, the volume discount applies to the total number of computer administrations
of the same SASSI software type ordered at one time. To attain the full functionality and
convenience of the SASSI software, a separate installation CD will be provided for each
user at an additional charge of $10.00 per user (to order additional user installation CDs,
please call customer service).
Upon installation, the software must be activated by calling customer service at
800.726.0526. Thereafter, additional computer administrations can be purchased over the phone and
activated within minutes by calling this same number.
For those interested in evaluating the software on a limited basis, a demo CD with
5 administrations is available for $25.00. Also, single or multiple administrations
of the Adult SASSI-3 can be purchased and administered online by registering at
www.sassionline.com.
Installation software is shipped on CD media. If you would like to order disks,

SASSI Online (www.sassionline.com)
The Adult SASSI-3 can now be administered online at any location with a computer
that has an Internet connection, such as your office, the client's home, etc. Single
or multiple administrations can be purchased starting at $12.00 per administration with
discounts for larger quantities.
SASSI Online provides registered users:
- Client notification by e-mail for in-home or remote administration
- Results within minutes, including the decision rule outcome, a graph of the client's scale scores, and a brief interpretation of the client's profile
- A secure online workspace to view SASSI results
- Pay-as-you-go flexibility
Web-based administration of the Adolescent SASSI-A2 will be available via www.sassionline.com
at a future date.
